feat: Add UserStatusBadge showing subscription level and credits

New component in header shows:
- Subscription plan badge (Starter/Bronze/Silver/Gold with icons)
- AI credits balance with sparkle icon

Also includes SQL migration for user_subscriptions RLS fix
